Details

The drug acitretin is a metabolite of earlier antipsoriatic retinoid, etretinate. This medication works in psoriasis by slowing down the proliferation of the skin cells. A response is seen in more than half of treated patients. About two weeks after starting of treatment, improvement begins and is maximum after about twelve weeks. Some patients are treated for a few months, repeated from time to time, while others remain on the acitretin long term.

Dosage:

The therapy with Soriatane should be initiated at 25 to 50 mg per day; it should be given as a single dose with the main meal. After it the maintenance doses of 25 to 50 mg per day may be given dependent upon an individual patient’s response to initial treatment.

How to use:

The medication guide should be read carefully and the medication should be taken by mouth once in a day with the main meal. It can take 2 to 3 months for full benefit to be shown. The medicine should be taken same time each day to get full benefit.

Uses:

This medication is a retinoid used in the treatment of severe psoriasis and other skin disorders in adults.

It is used in the treatment of extensive granuloma annular, severe ichthyosis, extensive actinic keratosis and skin cancers, extensive warts.

Drug interactions:

The drug Acitretin should not be taken with the following kind of medications:

  • Vitamin A or any other retinoid e.g. isotretinoin
  • Tetracycline or one of its derivatives
  • Methotrexate
  • Phenytoin
  • If triglycerides levels are high, it is best to avoid alcohol.

Contraindications:

The drug is contraindicated in the following cases:

  • In patients who have severely impaired liver or kidney function and in patients with chronic abnormally elevated blood lipid values.
  • The combination of methotrexate is contraindicated with this drug because increased risk of hepatitis has been reported.
  • Combination of tetracyclines is contraindicated with this medication because it can cause increased intracranial pressure.
  • In case of hypersensitivity this drug is contraindicated.

Precautions:

The precautionary measures that are to be taken into consideration while the use of this medication is:

  • If the person is allergic to the medication or to other retinoid, it should be clearly told to the doctor.
  • The medical history of patient like kidney disease, liver disease, heart disease, diabetes, receive phototherapy should be clear to the doctor before the start of medication.
  • After stopping therapy, the blood should not be donated for at least 3 years.
  • Prolonged exposure to sun, tanning booths or sunlamps.

Side effects:

The possible side effects of the medication are:

  • Blurred vision, headache or pain behind the eyes, nausea, vomiting
  • Sudden decrease in night vision
  • Depressed mood,aggression,unusual thoughts or behavior, thoughts of hurting yourself.
  • Jaundice(yellowing of skin or eyes)
  • Loss of feeling in the hands or feet
  • Dry reddened skin, apply moisturizers
  • Dryness of lips, apply lip salve frequently especially when outdoors.
  • Increased susceptibility to sunburn, protect the skin from sun.
  • Aggravation of skin complaint, this is often temporary and  followed by improvement but if severe flare occurs, it should be told to the dermatologist and stop taking the acitretin.

Overdose:

Overdosing of the medication should not be done. If any overdosing symptoms found, immediately visit to the nearest hospital.

Missed dose:

The missed dose is taken as soon as get remember. But if it has been the time of next dose, the missed dose is skipped and the next dosing schedule is followed.
